dental sealant program

Attention VALLEY parents of 2nd and 6th graders:

 

The Jackson County Health Department will be coming to Valley Elementary and Middle School to provide FREE dental screenings and preventive dental sealants for 2nd and 6th graders. The PURPLE authorization forms will be going out the week of January 9th so be looking for those forms to come home. Please return the form as soon as possible, completed, including YES or NO box, and your signature.

 

If a participating student presents any issues or concerns found by the JCHD dental team, this information will be sent home to the parents, as well as, possible providers to obtain treatment. Any healthy adult molars your child has, including 6 and 12 year molars, will be provided with dental sealants to help prevent decay in these important teeth. Any sealants provided will be communicated to parents as well.

 

If your child has no insurance or private insurance, you will not be billed and your benefits will not be used. If your child has Medicaid, they will cover these services and the parent does not need to do anything except provide the Medicaid number on the form. The JCHD dental team takes care of the rest. There is NO COST to any student or parent.
